Family Health Tracker, LLC believe that your health data is personal and owned by you, so we treat your health data carefully and securely.

Personal Information and Other Information or Data We Collect

1. Personal Info Stored and Required or Requested for Full Program Use – this information is collected in pre-defined fields and is used by various program functions.
   • Information Required such as your name, address, date of birth, gender and email address.
   • Information Requested such as your phone number(s), blood type, drug, food or other allergies, emergency contact info, insurance carrier and policy number (insurance info is available for use on children school-sports reports).
2. Personal Info Not Required nor Requested for Full Program Use
   • We do not collect Social Security numbers, medical record or patient ID numbers.
   • If you enter such information into your FHT record it is not used by the program.
3. Non-personal info (browser type, IP address, etc.) That May Be Stored
   We may collect non-personal information such as browser type used (e.g., Internet Explorer or Firefox), or the IP Address used to make a connection, to aid us in making program improvements or providing technical support.

Uses of Information

1. We use the information you enter to provide the services you license from FHT, including information about your condition, your medications, your schedule, providing you notices, etc.
2. How we do not use the information you enter:
   • Provide to any other party (except to a government authority with the recognized right to access your records) without your express permission such as when you decide to provide the information to others either through print outs or electronically or by providing others access to your account(s).
   • We never provide the information to pharmaceutical companies, insurance companies, employers, or any advertising or marketing companies.

Safeguarding Information Collected

1. We have physical, technical and procedural safeguards to help protect your personal information which is stored on our protected computer equipment.
2. We limit access to your personal information to employees having a reasonable need to access this information. Such access is subject to the safeguards mentioned in this section.
3. The data (information) that makes up your Service Account(s) are protected with industry standard encryption processes.
